About Community Health Foundation

 Who we are  
 Community Health Foundation, Inc.(CHF) is a non-profit organization. We are a member of Community Health Services of Georgia, Inc.(CHSGA) system of integrated healthcare delivery.  The Foundation’s mission is to serve others through positive change by:
  1. providing ministry and support to organizations who are members of CHS and their associates
  2. facilitating individual involvement in community projects; and
  3. helping to shape and model a more positive corporate culture.
 
 Our Mission
Community Health Foundation, Inc. is a 501(c)(3) non-profit organization. It is a member of Community Health Services of Georgia (CHSGA). It is the mission of Community Health Foundation to make a positive difference in the lives of all who have contact with us. We seek to accomplish this by: 1) providing ministry and support to organizations who are members of CHSGA and their associates; 2) facilitating individual and organizational involvement in community projects; and 3) helping to shape and model a positive corporate culture through the teaching and modeling of core values.

Community Benefit  
Community Health Foundation is a member of Community Health Services of Georgia (CHSGA). CHSGA as a nonprofit organization, has undertaken an internal study designed to assess, develop, enhance and report on the community benefit programs of the System and of each member thereof. CHSGA intends to rely on the 2006 Guide for Planning and Reporting Community Benefits published by the Catholic Health Association of the United States until more definitive guidance from the IRS is available.  


Code of Ethics
As a matter of fundamental principle, Community Health Foundation, a not-for-profit organization, is committed to embracing and implementing ethical standards. We do not take lightly the trust that has been placed in us by others to carry out our mission, to be good stewards of our resources and to uphold standards of conduct.

Our organization has adopted a Code of Ethics that includes

  • Our Code of Conduct

We are committed to the development of systems to “live out” our Code of Ethics. It is our goal to promote an organizational culture that encourages ethical conduct, promotes compliance with the law and supports activities which advance our mission which is to make a positive difference in the lives of all who have contact with us.
