Our History

Community Health Foundation, Inc. (CHF) is a Georgia non-profit corporation formed in March 2003 as a member of Community Health Services of Georgia, Inc. (CHSGA), an integrated health care delivery system. Formed to carry out both internal and external charitable projects, CHF has three goals: 1) to operate a scholarship program for educational assistance to students interested in the fields of nursing, nursing administration or other approved healthcare fields of study; 2) to provide financial support to other members of CHS including supportive services to patients and customers who have unmet needs; and 3) to support creation and development of new health care programs designed to meet the needs of residents or patients of the System.  Wrapping Presents
Community Health Foundation was formally organized in 2003, but its roots are much deeper, as its precursor was Health Scholarships, Inc., organized nearly 15 years ago. Since 1989, the organization has funded over 125 scholarships to deserving students who were committed to health care careers. The Foundation continues that mission, but the Foundation’s net is cast wider to provide additional support to communities in which the System operates. 

CHF is a charity that depends upon the public for its support. Community Health Foundation is committed to linking with local community institutions and organizations to address local needs through innovative partnerships that benefit both the System and the communities we together serve. These relationships also allow us to develop the Foundation as we identify needs in the communities and work together on charitable projects.

The Foundation participates in a variety of community charitable efforts, including employee participation in Habitat for Humanity, as well as other local projects in the communities served by the System.
